Your nutritionist/health coach takes a deep dive into your nutrition history and work alongside your clinicians to create a plan that fits your life, builds on your strengths, and supports meaningful, lasting change throughout your care at CCFM.

This is where we put it all together for you. At the Case Review, we review your lab results and your health from a functional medicine perspective to further personalize your comprehensive wellness plan and determine the next steps.

Implement Your Personalized Care Plan

Launching your comprehensive care plan can be complex, we know this from decades of experience. For optimal success, you will engage with our comprehensive support resources, including community classes, Q&A sessions, and both individual and group coaching. Reach out to your care team as needed.
